Journey with B23

Step 1

Refer Child

Fill out the referral form or give us a call to start the journey with B23.

Step 2

Evaluation for Eligibility

A multi-disciplinary team will conduct an evaluation with the parents and child. We offer a convenient setting for this appointment, such as their home, child care center, or a B23 program designated site.

Step 3

Team Up

Once the child is determined eligible, the family and their family service coordinator will complete assessments to help determine the unique strengths and needs of the child and family.

Step 4

Make a Plan

The family and their early intervention team collaborate to create their Individualized Family Service Plan (IFSP). This document outlines the child and family desired outcomes and early intervention services.

Step 5

Provide Services

The family will work with early intervention providers to achieve the child and family’s IFSP outcomes.
